Credit Card Verification

All credit cards MUST be verified prior to receiving a payout.

To verify your credit card, please complete the following steps:

1. For each credit card, enter the account information and print the Credit Card Authorization Form. We only require this information once per each card used.
2. Sign and date the bottom of the agreement.
3. Fax (1-800-887-8691) or email (cashout@vip.com) the Credit Card Authorization Form along with:

A photocopy of your picture ID (e.g. Drivers License, passport)

Photocopies (front and back) of your credit card

To avoid processing delays, please ensure that the name, address, card number and signature on your submitted documents are clearly legible. If you wish to do so, you may blank out all but the first and last four digits of the card card.
